If you are a male in the U.S. between the age of 40 and 59, you have a 50-50 chance of having an enlarged prostate. At age 65, almost all U.S. men suffer from prostate enlargement or BPH (Benign Prostate Hyperplasia). The prostate is a gland the size of a chestnut that weighs less than an ounce- that plays a major role in the male reproductive system. All during a man’s life the prostate continues to grow. So long as it grows outward, it causes no problems and no noticeable symptoms. However, if it starts to grow inward – into the walls of the urethra – it begins to obstruct the flow of urine. This condition is known as Benign Prostate Hyperplasia.

Enlargement of the prostate gland leads to a narrowing of the outlet of the bladder and urethra (the tube carrying urine from the bladder) with a resulting inhibition of urine flow from the bladder. The common signs and symptoms of BPH include painful urination, straining to urinate, decreased force and caliber of urinary stream, prolonged dribbling after urination, sensation of incomplete bladder emptying, to down right depressing impotence.

While the exact cause of BPH is unknown, most of the evidence points to hormonal changes that occur in middle-aged and elderly males. The role of dihydrotestosterone (DHT) – an extremely active form of testosterone found in high levels in the prostate tissue of BPH suffers. DHT stimulates enlargement of the cells of the prostate gland at a far higher rate than all other androgenic (male) hormones. This enlargement leads to BPH.

Looking more closely at DHT, we find the culprit to be an enzyme known as 5-alpha reductase (5-AR). 5-AR is responsible for converting testosterone to the more active DHT. 5-AR increases with age and its primary focus is the prostate gland.
