APPLICATION AND ORDER FORMS
An application form must be filled out completely, i.e. Name, Street, Apt. No., City, State, Zip, Social Security No., Telephone No., Sponsor's Name, Subscription Fee, Dated, and Applicants Signature. This form must be completed and mailed to the home office before a person can purchase product wholesale. All distributors must purchase a product manual. The distributor agreement must also be signed and dated by the new distributor. Please, please print legibly. The white copy is sent to the home office. The yellow copy is kept by the new distributor. The pink copy is sent to the new distributor's sponsor. This is an annual agreement and must be renewed yearly to keep your downline.
An order form must be completed each time an order is placed. Remember to include your sponsors name every time you fill out an order form. Please, please print legibly. The white copy is sent to UNLIMITED. The yellow copy is kept by the distributor. We will not accept copies of the order forms.
